Referring your patients to Dr Conrad ensures they receive expert, patient-centred care tailored to their individual needs.
Dr Dean Conrad welcomes referrals from General Practitioners (GPs), specialists, and allied health professionals.
How to Refer
Referrals can be submitted via the following methods:
Phone: Please call our rooms on (02) 9526 7477 to discuss a referral or arrange an appointment for your patient.
Fax: Send referrals to Fax: (02) 9526 7455. Please include all relevant medical history, test results, and imaging reports.
Email: Referrals can be sent to info@drdeanconrad.com.au. Ensure all documents are attached securely in PDF format to protect patient confidentiality.
Electronic Referrals: Our practice is set up to receive electronic referrals via HealthLink or Argus. Please contact our office for further details on setting up electronic submissions.
Urgent Referrals
If your patient requires urgent attention, such as those presenting with severe symptoms or potential malignancies, please notify our staff when submitting the referral. We will prioritise these cases to provide timely care.
Collaboration & Updates
As a referring practitioner, you will receive comprehensive updates about your patient’s progress, including diagnostic findings, treatment plans, and surgical outcomes. This ensures seamless communication and continuity of care.
